New requests!!

Ok, Bad news, the folder on my computer that I had saved all of my stuff too (Including all three requests I got from the people in the PPG club) got deleted. so, I've decided i'm going ot do new Computer generated ppg requests, to save up time on making them.
Its just orginal style this time because i'd have to use a base to do Doujinshi.

Heres the new form to fill out :
