In the promo it looks like Blair chooses Chuck over Louis and decides to run away with him in the limo and there are a lot of paparazzi cars chasing after them. That's probably what causes the car accident. Also, Blair is supposed to miscarry in 5x10. Chuck will probably get hurt in the car crash too :\

I don't think they will die. The show cannot afford to lose Chuck 또는 Blair. I feel bad that she will lose her baby, but the father was actually Louis so that's a win-win for Chair and Dair. No royal baby = No 더 많이 ties between Blair and Louis.
